Health Information for the 2011-2012 School Year
All students entering Prekindergarten, kindergarten, and sixth grade must have a physical exam completed with in one year of the start of the school year.
All students transferring to Community Consolidated School District 180 also must have a physical exam within one year of the transfer.
All students in kindergarten, second and sixth grade must have a dental exam completed by May 15, 2012. All kindergarten students must have a vision exam completed by October 17, 2011.
Information regarding the sixth grade physical and dental exams was sent home with fifth grade students in late May.
Information regarding the second grade dental exam was sent home with first grade students in late May.
All students at Burr Ridge Middle School who will be participating in extracurricular activities, such as football, volleyball, basketball, softball, cross country, tack, swimming, cheerleading, flag squad, wrestling and, etc. must have a sports physical. A sports physical is only good for one year from the date of the sports physical.
Dr. Higgins will be doing sports physicals for Burr Ridge Middle School students on Friday August 26, 2011 from 8 am to 12 pm. More information on the sports physicals will be available during summer and when school begins.
